Fertilizers is applied to the soil of the entire 5x5 plot it's used on, meaning it affects a 5x5 area which you can see by holding a [[Land Claim Stake]].

Each unit of nitrogen, phosphorous or potassium in the fertilizer corresponds to 4% of the corresponding nutrient in the soil (shown by the [[Soil Sampler]]).
Changes in the nutrient values of the soil take some time to update and will not be visible when checked with the soil sampler for ~10 minutes. You can add as many fertilizers to a plot as you want, but you will have to wait for the plot to update before you can read the nutrient change to that plot (as of v0.943).
